South Park: The Complete Fifteenth Season is a 2012 Blu-ray/DVD release of Season Fifteen, including all fourteen episodes from the season and featuring audio mini-commentaries by Trey Parker and Matt Stone. as well as deleted scenes, behind the scenes of "City Sushi", and the featurette 6 Days to Air: The Making of 'South Park'.

The background color is light blue and the title at the top, as opposed to the bottom in Season Fourteen. This is also the fourth back cover to not show all the four boys.

The front cover depicts Eric Cartman with his stuffed toys and the boys in their gym uniforms, referencing "1%". The back shows Kyle as part of the HUMANCENTiPAD, a reference to "HUMANCENTiPAD".

The inside cover depicts Eric Cartman armed with a gun, as the boys and Tolkien watches him, another reference to "1%". The back shows Lemmiwinks, Frog King, Sparrow Prince, and Catatafish, referencing to "Bass to Mouth".

Opening the inner case shows the wedding of the Prince and Princess of Canada, referencing "Royal Pudding", and a full-length depiction of Butters being cheered by Mexican civilians, referencing "The Last of the Meheecans".

The three discs shows the boys, Butters Stotch, Craig Tucker and Clyde Donovan celebrating Stan Marsh's tenth birthday and Kenny McCormick's parents being arrested by cops. Each disc references "Crack Baby Athletic Association", "You're Getting Old", and "The Poor Kid", respectively.
